Prescription Request

If you have seen the doctor in the last 6 months, you may order a repeat prescription by using our Patients’ Portal online or the APP.  For more information, please go to our App information page.  You can also fill in the form below. The Doctor will send your prescription to your chosen pharmacy, please allow up to 48 hours. If you require an urgent same day prescription, the charge is $35.

Sometimes an appointment with your doctor or nurse is necessary to re-assess your condition. In that case, you will be contacted by email, text (SMS) or telephone.

Please check prescription fees here. Urgent same day prescription charge is $35.  Payment is appreciated within 7 days and can be made by internet banking or at the surgery. Late payment will incur an accounting fee.
